MIDI timing errors when changing tempo while playing

Here is a video (1.1mb QuickTime .mov) showing a few issues with MIDI timing when changing tempo while playing, by mouse wheeling over the BPM control on the transport.

As you can see, there are a couple of severe problems:

(1) The order of notes gets messed up: the loop plays a low note, then three notes an octave higher, like a metronome. But that is not what we hear or what the log shows.

(2) For some notes there is no corresponding note-off.
