Default REAPER User Guide: massive update for 4.5

Congratulations to J, C, S and the beta crew for an amazing update ... surely the biggest and best dot release ever!

Great software needs good documentation, and I hope the latest upgrade to the User Guide will deliver this. This release does wonderful things for REAPER's MIDI capabilities (especially the MIDI Editor), but there's stacks of goodies for non-MIDI users too!

In addition to numerous tweaks, twiddles, fixes and minor updates, the main additions to the new User Guide are:

Improvements in/additions to Performance Monitor.
Project Tabs: Option to show on left.
Autosave: new options.
Summary table of supported media formats (now includes .WMA).
New option: Set grid to frame (rather than beat).
New section: Hardware FX monitoring.
New section: Insert space at time selection.
Dynamic splitting: Action to perform list added.
Option to replace stretch markers with dynamic splits.
Changes to MIDI Filter Events dialog. New Snap to Grid Options.
MIDI Editor: 14-bit CC messages/lanes.
MIDI Editor Options menu: new multiple media items options.
MIDI Editor Options menu: new Note preview options.
MIDI Editor Options menu: Allow MIDI item edit to extend media item.
MIDI Editor Options menu: CC events in multiple MIDI media items options.
MIDI Editor Contents menu: various changes for working with multiple items.
MIDI Editor: 14-bit MSB/LSB CC data display options.
Working with multiple MIDI items: new features.
New sections: Managing multiple MIDI tracks and items.
New MIDI track list and MIDI item lane.
New section: Editing multiple MIDI items.
MIDI Editor Actions list and editor.
New section: Feedback routing with MIDI tracks.
New section: Working with MIDI editor synced to project.
Audio Settings: Specify output for metronome.
Changes to MIDI Editor preferences.

I've made several mostly subtle changes to document layout to stop the document length from getting ridiculous: these shouldn't impact on clarity or readability (I hope!).
