Default Control reaper actions with cc# using a vst as the controller

Hi, I know how to map cc controls to actions, and I use this with my keyboard already. However, I am using this MIDI pads vst and I want to map the pads and sliders to actions.

When I send cc to reaper from an external MIDI source, it triggers the action fine. However, cc sent from inside a VST doesn't seem to trigger an action.
Now, I could just create buttons in my toolbar, and I might end up doing that. However, I can't create sliders this way.

Is there a way to link VST controls to actions? Alternatively, is there a kind of controller slider I can set up in Reaper without a VST?

It's a shame that something so uniquely important, obviaous, and simple as MidiToReaControlPath neither comes with the Reaper installation by default nor is available via ReaPack
