You can do that with (P)roject(i)n(P)roject, not the most convenient way if you are working with thousands of individual media items, but I bet PiP could be streamlined to make "item markers" a simple thing.
Otherwise, yes, we NEED item markers. I'm constantly running into situations where item markers would be highly useful.
