Customize Toolbar window

A few things:

(a) Add a feature to collapse / hide the Default menu on the right-hand side so that the Customised menu lines can be read in full and the window kept smaller. The view view is:

(b) When a text icon is entered, show the default name too in the list

(c) use the full width of the window for previewing the toolbar and also change the height of that preview window by dragging a divider up/down

NB the preview does not always reflect the actual toolbar (raised as a separate Bug report http://forum.cockos.com/showthread.php?t=106833 )
