two instances of plugin causing the host to crash

I'm running a beta of a plugin I'm working on, and I'm getting some reports of two instances of the plugin causes the host to crash (reaper/cubase..WinXP SP3/Win7)...

I'm asking because the project is pretty large and it doesn't happen on my computer and I'm looking for tips to watch out for..
(I have no global variables in my plugin)

I noticed this with the bass plug. Put another instance and the vu pegs and that track stops audio.. Especially if you drag and drop from one track to another.
