Default Mouse modifier settings displaying/acting weird

Problem: weird/inconsistent display of mouse modifiers. See pic below.

The problem isn't just visual though - it also makes selection weird too (right now, every time I select 'move edit cursor' it also selects 'hand scroll', which can't be unselected) (just visual)

How to reproduce: just edit the relevant mouse modifier settings above and it should happen. If people can't reproduce it I'll give more detail about my setup.

Seeing this in Reaper 5.211 (don't know about previous versions)

I think that is working as intended, "Hand scroll and horizontal zoom" being the choice, the subchoices just set how it behaves in addition to hand scroll.

Right, right, that explains the behavior (post revised). But the display is definitely messed up. See the second pic? Invert should be ticked. And pic 1 doesn't have handscroll ticked, but sometimes it is ticked.

Hmm, actually you are correct, and it's possible to untick "Hand scroll" to get the other actions without it. And the "inverted" option selection acts wonky, confirmed.
