Latency giltch when recording

Hello all

I've been having an issue pop up with the last few versions of Reaper across a few different OSX machines. I have a windows machine but I don't really use it for recording so I haven't noticed if it happens on that.

Occasionally when recording there will be a glitch/pop sound and after that the live input will have a ton of latency (like more than 60ms)
If i go into the Audio Devices and reinitialize the buffer settings it goes away, but this is a big issue as it's happening on sessions and ruining takes.

This happens maybe 1 - 3 times in a 10 hour session, but it's happening on 3 different computers:
-A Hackintosh running yosemite and a focusrite pro 40
-a 2011 macbook pro with a usb mbox
-a 12 core trash can macpro at a studio I work at often with firewire UAD Apollos.

I'm having a very hard time figuring out why this is happening... I havne't noticed any particular pattern, has happened when recording anywhere from 1 track and upwards of 10 at a time.

Could it be a plugin I'm using? A script that is running?
Anyone else experienced this problem?
