Ive done so many searches looking for an answer to this but just cannot find anything
Im looking for a way to automate the volume with an LFO frequency that that changes over time from say 12hz to 15hz as an example
Is there a way to do this in Reaper ?

it's all just maths, so you could work it out as automation items give you the number of cycles.
It would be cool if you could modulate or automate Parameter Modulation parameters though. There's probably a feature request for it.
I worked out a way to do it with two tracks and a MIDI LFO (second video) but that seems fiddly and completely unnecessary work when a tremolo effect is an LFO that you can automate volume with to precise Hz like this:
This seems silly in comparison to the above:

Hz is cycles per second, automation items give you the number of cycles per item, so couldn't you just do a calculation based on the length of the item? I'm not sure how the frequency skew would work though, maybe have an automation item of your desired freq to start with, and another auto item with your desired freq to end and then maybe adjust the skew (of the first item) by ear.
Admittedly this is probably a hassle, but just throwing out there, someone else probably has a better solution

I was doing a very basic version of this with the JS tremolo
But I’ll download the one you’re showing for a bit more editability

I messaged someone this morning about making something specific for me in JSFX, so waiting to hear back
I don't know if the Melda one is free, I just grabbed it because it was the first one I saw. Didn't have a chance to try the JS tremolo but I think at the end of the day, tremolo may be the easiest choice if it has enough control over the LFO shape and other params. A quick test showed JS:tremolo working pretty good to me.
A side note that you can right-click an env point and choose Set point value for precise frequencies - maybe obvious, just mentioning.

The volume was just the starting place really
Then was gonna move to panning and see what other fx parameters made sense musically
I like the free Pecheneg tremolo and automate that either speed in Hz or host beat (via bpm+note), depth of effect and you also have control over the waveform, phase of it and symmetry. Interestingly you can also switch to be a panner as well (just saying, since you mentioned pan!).
