Default VST presets changing/re-setting

I've tried to search for this in the forums without success. I experience problems when I assign a VSTi FX to a track. The preset that I select changes to a different preset as soon as I hit play. I've tried different VST instruments and many but not all do the same thing - revert to a different preset other than what I had selected as soon as I click play.
Thoughts?

Default VST problems

I've discovered the issue - if you have a VSTi with more than 41 presets, Reaper only defaults to the 41st preset. So if you like the 4th preset, when you hit play it automatically replaces #4 preset with #41 - very annoying. This does not occur is your VSTi has less than 41 presets.

Which plug-in?

Perhaps you could compress a simple problem project file (no audio samples needed) into a ZIP file and post it here as an attachment so that we can have a look at it and see what's (not) happening?
