Default Asking for help to troubleshoot focusrite 18i20 and midi throughput

I'm having issues with midi throughput being so slow that even when I set focusrite latency to maximum I am getting lots of "stuck" notes. Seems to have started with Reaper 5, but that may be coincidence.

I am still running an old dinosaur computer, but it's handling everything I throw at it just fine until lately with the midi.

Intel Core2 quad @ 2.4ghz
Yamaha S80
focusrite 18i20 (1st gen)
Reaper latest version
Sampletank 2.5
Vista 32bit

If I create a sample reaper project with one track containing sampletank vsti, assign an instrument to a midi track, and output that instrument's audio to a 2nd reaper track, latency is fine.

By the time I get to 16-24 audio tracks, and add 5-10 more midi instruments, midi latency is terrible. However, last night, on a typical project as described, my cpu was rarely hitting 10% usage.

I'm asking for items to troubleshoot, or settings to change. I've had no luck finding a bottleneck. C-state and EIST are not even an option in my old bios.
