"=" and ":" can cause problems when saving files
I used to name tracks after mics I've used, for example I recorded vocalist with 2 mics, and name of file was like "L:Sennheiser441 R=SeZ3300A". When loading session again, files couldn't be read, I needed to load them "by hand". I guessed the culprit must be some kind of code injection when using "=" and ":". I avoid using those symbols in track and filenames, and don't have this issue anymore.

I'd imagine that, if you're on Windows, "L:Sennhsiser441" will be interpreted as a file in the current directory of drive L:

Other programs won't let you save, e.g.:
So I think what is needed here is an error box when using certain symbols. / and \ also can't be used (they cause a folder to be created)

That's right and what you should always do. Those are illegal filename characters at the OS level. Don't use them, for filenames, ever. The GUI should likely warn someone who is unaware but you are now aware. The built in OS dialog will prevent you from doing this so it sounds like the dialog in Reaper is custom written and needs an illegal chars for filename check.

Windows seems to have a lot of restricted characters for file names that are perfectly fine in OSX. That's probably not going to change any time soon so if you want to share files it's best to avoid punctuation in file names.

Best practice, I think, is to use unix-like file naming: alphanumeric, _, and -, no spaces, using capitalization only where necessary, or for camel-case, i.e., someFile.txt.
