Window menu displays multiple "Enter Full Screen" (OS X) (FIXED)

Steps to reproduce:

Work for a while on a project
Check the Window menu

Result: There are multiple "Enter Full Screen" menu items, sometimes more than 10

Expected: One "Enter Full Screen" menu item

Every time you open the GUI of a plugin effect, this gets added. If you're working a lot with VSTi's or effects, this gets out of hand real fast.

+1; please fix this.
