There appears to be an issue with MIDI files where certain sections will become slightly off of what they were authored as upon export, but after a few beats, everything snaps back into place. I first mentioned it in this thread here:

Hello,
I have an extremely weird thing happening with a MIDI export for this one particular MIDI file.
Before an export, my MIDI looks like Image 1, all notes are snapped to the grid. However, when I export it, a few notes starting at measure 203.3 get what looks like time-stretched and are off of what the original MIDI was until measure 204.2 where everything is snapped to beat again. This can be seen when I re-import it (Image 2).
This is the only part of this MIDI where that happens to, the whole rest of the file is exactly as it was in the original MIDI file.
My Midi export settings are:
- Entire project
- Selected items only
- Multitrack MIDI file
- PPQN: 480
- Embed project tempo/time signature changes
I'm running REAPER v7.01, but this also happened in v6.
Has anyone run into this before? Is there a setting that I am missing? Please let me know.

I went back and found the exact version this issue is introduced, and it's
v6.81. The issue has persisted until the current release.
Here are the images from that post:
The original MIDI file:
Measures 203 and 204 after exporting and re-importing:
Something I didn't mention in the other post was that it affects all events in those three beats, they all get shifted by the same amount.
Please let me know if there's anything you'd like me to try, but it seems to be consistent. I unfortunately don't know how to replicate it in other MIDI files, so if needed, I can share the MIDI file it's happening to me with.
